Summary:
Educational Service District is looking for a qualified individual to fill the role of the YVP Care Coordinator.
This position is responsible for assists youth in the community that present with risk factors associated with violent behavior.
The Care Coordinator will provide direct service to youth and their families by navigating community resources to increase protective factors and decrease risk factors.
ESSENTIAL FUNCTIONS (to include but not limited to):
Assess youth and family needs to establish community/educational goals.
Assists in developing education plans for transitioning students suspended or expelled from school for the purpose of finding an appropriate school placement.
Meet with students and families, counselors, school officials and others to facilitate school enrollment and coordinate job placement for the purpose of facilitating school enrollment and coordinate job placement.
Monitor progress of care coordination goals.
Participate in a variety of meetings and travels both locally and statewide (e.
g.
conferences, meetings, trainings) for the purpose of gathering and conveying information regarding youth.
Serve as a liaison between education programs, Juvenile Justice system, law enforcement and other related services.
Maintain contact with administrators and other staff of school districts and educational programs within each county; acts as an advocate of transition students; provides consultation and training regarding delinquent youth to education program staffs as needed; may serve as liaison between parents and education programs.
Attend meetings as requested by each school district and detention center or as required by program needs.
May sit on various advisory committees as required.
Attend Multi-disciplinary Team meetings for youth on caseload.
Complete reports and other required data tracking, provides student progress reports to professionals involved in the care of the youth.
Maintains confidential casebook, records, files and information as required.
Maintain knowledge of available educational support services and programs; provides information regarding community services available to youth and parents.
Conduct screenings to identify the needs of students and for referral.
Perform related duties consistent with the job description and assignment.

Qualifications:
Bachelor's degree in psychology, sociology, education, or criminal justice; or a two-year Associates degree with a minimum of two years' experience working with local community services or government agencies regulating social and/or health services, juvenile justice, or rehabilitation.
Experience working with at-risk youth, preferably in a community setting.
Experience in case management, documentation, and record keeping.
Minimum of 2 years of experience working with adolescents.
Demonstrated case management / record keeping skills.
Demonstrated knowledge of alcohol / drug abuse federal confidentiality laws.
Demonstrated knowledge and understanding of substance abuse assessment, intervention, treatment, and recovery process for adolescents and families.
Demonstrated knowledge of violence as it relates to bullying and harassment.
Demonstrated verbal and written skills.
Willingness to take direction from supervisors and understand ESD policies and procedures related to employee responsibilities.
Ability to work successfully as an ESD team member by contributing to the ESD Strategic Plan, including supporting agency initiatives and sponsored events.
WSP/ FBI background clearance required.
Requires knowledge of:
